ANY ADDITIONAL COMMUNICATIONS BETWEEN YOUR HOME HEALTH STAFF AND PHYSICIAN ARE THROUGH YOUR ADVOCATE, YOUR CASE MANAGER.

Our case managers work with patients and their families to create a plan of care that works with your personal network and is approved by your physicians.
