Medical Social Work

Our medical social workers assist our patients and their families with short-term counseling and links to community service, including:

  • Patient assessment
  • Referrals to community resources
  • Nursing home and institutional placement assistance
  • Assisting patient, family, and team members with social, emotional, and environmental issues relating to the patient’s health problems
  • Consulting with the physician regarding the patient’s status if well-being is adversely affected by social, environmental or economic conditions
  • Providing rehabilitative and supportive case work services
  • Development, implementation, review and revision of the plan of care

Home Care Aide

Our Home Health Aides work under the direct supervision of a license professional, such as a Registered Nurse Case Manager. Home Health Aides are responsible for documenting the care provided to your loved one, and notifying the case manager of any changes in health. Home Health Aides may assist in the following areas, but not are not limited to:

  • Administering medications
  • Bathing, washing hair and shaving
  • Assistance with dressing
  • Getting in/out of bed
  • Feeding and meal preparation
  • Maintaining a safe living environment
  • Monitoring blood pressure, temperature and other vital signs
