5 Steps to Prevent Falls:

  1. EXERCISE to build balance, strength and flexibility.
  2. CONSULT with your healthcare physician about getting a fall risk assessment, including postural hypotension. Home Health Professionals can provide safety evaluations in your home.
  3. REVIEW MEDICATIONS with a doctor, pharmacist or your Home Health Professionals team to ensure side effects don’t increase fall risks.
  4. GET VISION AND HEARING CHECKED ANNUALLY and update eyeglasses.
  5. KEEP YOUR HOME SAFE and remove tripping hazards, increase lighting, make stairs safe and install grab bars in key areas.

Care Transitions

Designed to improve the transition process as a patient moves from an inpatient setting, such as a hospital or rehabilitation facility, back home. Through our Care Transitions program we work closely with patients to assist in managing their disease process or rehabilitation with a goal of reducing the risk of the patient being re-admitted back to an inpatient facility.

  • 20% of Medicare beneficiaries are hospitalized within 30 days of being discharged1
  • 50% of patients that were re-hospitalized did not see a physician in between their hospitalizations1
  • Up to 76% of readmissions that occur within 30 days of a hospital discharge may be preventable2

Diabetes Management

Home Health Professionals’ specialized Diabetes Care Management Program helps patients manage diabetic care in the comfort of their own home. Our highly skilled nurses work directly with the patient’s physicians and families to provide support, education and assistance to diabetic patients.
