PHYSICAL THERAPIST (PT)
The Physical Therapist is a qualified professional who assists the physician in evaluating level of function, helps develop the plan of treatment (revising as necessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel, and participates in in-service programs.

Job Description: We are seeking a highly skilled and motivated Home Health Physical Therapist (PRN) to join our team. As a PRN Home Health Physical Therapist, you will provide high-quality care to patients on an as-needed basis, helping them regain mobility and improve their overall well-being. You will work closely with patients, their families, and other healthcare providers to create and implement individualized treatment plans.
KEY RESPONSABILITIES
-Perform comprehensive assessments of patients; physical conditions and
develop personalized treatment plans.
-Deliver therapeutic exercises, functional training, and other interventions to
enhance patients; strength, balance, and mobility.
-Educate patients and their families on safe practices and home exercise
programs.
-Monitor and document patients; progress, making necessary adjustments to
treatment plans.
-Collaborate with other healthcare professionals to ensure coordinated and effective care.
-Maintain accurate and timely patient records in compliance with regulatory
standards.
–Provide compassionate and empathetic care, respecting the dignity and privacy
of patients at all times.
QUALIFICATIONS
-Valid state licensure as a Physical Therapist.
-Minimum of 2 years of experience in home health or related field preferred.
-Strong clinical skills and knowledge of various physical therapy techniques.
-Excellent communication and interpersonal skills.
-Ability to work independently and manage time effectively.
-Reliable transportation and willingness to travel to patients homes.
-Kinnser software trained preferred.
–Proficient in Oasis start of care documentation preferred.
